This might be because of slow network connection also.
U can also see NS log stating that
Warning: Failed connect attempt to agent<agent> is too slow. It took 130916 milliseconds to detect it can not be connected. This may slow down availability detection process (availability loop). This warning threshold can be controlled via configuration parameter WARN_SLOW_FAILED_CONNECT_MILLIS
Alternatively you can use ctmgetcm utility on CTM server to view/get detail of agent configuration , if this fails need to check cnnectivity between agent and server
try analyzing below report

from agent - ag_diag_comm
